What Community Health Centers Mean to Americans and How they Manage Chronic Disease

Community Health Centers have been providing quality primary and preventative health care to the nation's most vulnerable populations--regardless of their ability to pay--for over four decades. In 2011 they served over 22 million people in over 9,000 sites across the country. The community health center model of health care has proven to be efficient and successful in providing much-needed services to address chronic diseases generating $24 billion in savings to the US health care system.
